Frequently asked questions
How effective is ketamine for depression?+
Across published trials of treatment-resistant depression, ~50-70% of patients show clinically meaningful response within 1-2 weeks of starting treatment, and ~30% reach remission. Effect sizes are substantially larger than placebo in most controlled trials.
How quickly does ketamine work?+
Many patients notice mood lift within hours to a few days of the first session — faster than any oral antidepressant. Full benefit typically emerges over the induction series.
How long does the antidepressant effect last?+
After a single dose: typically 3-7 days. After a 6-session induction: weeks to months. With maintenance: indefinite for many responders. There's significant individual variation.
What predicts a good response?+
Earlier-onset depression, family history of depression (vs. trauma-driven), strong therapeutic alliance, healthy sleep and exercise, and active integration work all correlate with better outcomes.
What predicts non-response?+
Chronic benzodiazepine use, lamotrigine, untreated trauma without integration support, active substance use, and severe medical comorbidity all reduce response rates.
Do people respond to ketamine when SSRIs have not worked?+
That is the population most ketamine trials enrolled. Mechanistic difference means SSRI failure doesn't predict ketamine failure. About 50-70% of TRD patients respond.
How effective is ketamine for anxiety?+
Smaller evidence base than depression, but several controlled trials show benefit for generalized anxiety and social anxiety. Effects appear less durable than for depression and often require ongoing dosing.
How effective is ketamine for PTSD?+
Promising. Repeated infusions have shown significant symptom reduction in controlled trials (Feder 2021), particularly for combat-related PTSD. Best paired with trauma-focused therapy.
Does ketamine cure depression?+
No treatment "cures" depression in a permanent sense — recurrence is the norm. Ketamine produces rapid relief and supports a neuroplastic window for psychological work; reported durability of benefit varies widely between individuals and study designs.
What is typically done when a course does not produce improvement?+
In published protocols and clinic practice, a full induction (6-8 sessions) without meaningful improvement is usually treated as a signal to reassess rather than continue. Alternatives discussed in the literature include TMS, ECT in severe cases, intensive psychotherapy, other medication classes, and structured lifestyle programs. Which of these is appropriate is a decision for a treating clinician.
