Is ketamine 'just' an opioid?
No. Ketamine has weak affinity for mu-opioid receptors compared to its NMDA action. Whether that weak opioid signal modulates the antidepressant effect is unresolved — replication attempts have produced mixed results.

  • Williams 2018: Naltrexone (opioid blocker) appeared to prevent ketamine's antidepressant effect in 12 patients.
  • Yoon 2019, Klein 2020: Replication attempts found no such blockade.
  • Current consensus: Opioid system involvement is possible but not established. The glutamate cascade remains the dominant explanation.
Why this matters clinically
For people with opioid use disorder histories, the question isn't just academic. Some clinicians prefer to disclose the uncertainty and consider esketamine or alternative pathways where appropriate.
